The film explores a rather more private chapter in the life of the composer Giacomo Puccini: the suicide of his maid Doria Manfredi. Long believed to be his lover, the woman poisoned herself in 1909, at a time when the musician was composing "The Girl of the Golden West.".
As for the title, it simply means Puccini and the Girl, though I assume it is also a play on his opera La Fanciulla del West - in the movie it seems to refer to the servant girl he was having sex with. Gorgeous surroundings, servants galore, and everybody, even the children, are too unhappy to speak.
